Complete either or both application(s) as indicated
Academic Award only - include a writing sample (250-500 words) on the topic indicated on application.
Community Service Award -- on separate sheet explain total involvement in project also include flyer or newspaper clippings - if available.
Attach supporting documentation. The following items are the only items that will be accepted: current school transcript, most-recent grade report, and test results.
Two letters of recommendation from adults other than family members.
Send the application to the following address. Application must be postmarked no later than April 1, 2019 and received at the St. Louis office no later than April 5:

Hugh Cranford served as Executive Director of Optimist International from 1967 to 1986. He was instrumental in starting the Optimist Junior Golf program and in 1978 led the efforts to co-sponsor the Optimist Junior World Golf Championships in San Diego, the forerunner to today’s Optimist International Junior Golf Championships. Cranford passed away August 2014
